LICENCE RENEWAL
Renewal cadence: January 31 of odd numbered years.
Renewal fee: $68
Required contact hours: One of the following must be true: – 400 hours practice – Licensed practice for at least 200 hours and 15 hours of approved continuing education – 30 hours of approved education
The Utah DOPL will mail a renewal notice to your address on file 60 days prior to expiration
Nurses must also complete a suicide prevention training which can be foundhere
Important Things to Know
If you answered “Yes” to any of the questions on the Qualifying Questionnaire, additional documentation may be needed.
In order to apply for licensure by endorsement, you must have been licensed in another state for at least 1 year.
Request verification of licensure via Nursys from at least one other state you are licensed in. If this state does not participate in Nursys, you will need to contact that state’s BON and request verification be sent to the Utah BON.
