Position : Clinical Care Nurse

Registered nurse; 2 years of experience required.

Home health or community health experience desired.

Current Colorado license.

BSN required  (MSN encouraged)  

Current B.L.S. certification. 

Certification in case management (eg CCM, ANCC RN-BC, ACM) to be obtained within 3 to 5 years of hire date.

Certification in Motivational Interviewing (preferred).
