Position : Float Pool Registered Nurse
Graduate of an approved School of Nursing for Registered Nurse.
Current Vermont Registered Nurse license, BLS, and BSN required.
ACLS, PALS, NRP, Tele Course mandatory within one year of hire.
2-3 years clinical experience.
Develops a collaborative plan of care with the patient, family, and interdisciplinary team. Provides nursing input towards initial and updated care plans, assists in monitoring progress toward stated goals. Updates POC according not less than every 14 days and as needs change.
Manages assigned patients in the inpatient unit with comprehensive pain and symptom management according to the plan of care.
Provides comprehensive nursing assessment. Assessments include: symptom management, including, but not limited to: pain, bowel and bladder function, level of consciousness, alteration in nutritional and fluid balance, alteration in skin integrity. Updates the patient’s medication record.
Performs a pain assessment at every visit/ or minimally once a shift in the inpatient unit.
Communicates timely with the primary nurse and the appropriate team members regarding patient assessment and recommended and instituted care measures.
Maintains competencies in pain management, symptom management, infusion care, wound care, nursing assessment, medications, etc.
Performs in home/facility HA supervision visit per agency policy. Addressing: HA care plan meets the patient’s needs, HA provides care according to HA care plan and patient/family is satisfied with HA care. Documents the supervision.
Updates written HA plan of care when indicated.
