Position : Patient Liaison
Complete timely communication with family or significant other at the following frequency via phone:
Ensure a security passcode is established for the patient’s contact person. Document passcode under team sticky note in Epic.
Help set reasonable expectations for future communication
Designate an emergency contact. This will be the point person for all family updates.
Introduce Nurse Provider Rounding process.
Communicate status upon admission to the unit.
Communication points with contact person
Daily call to inform contact person of patient’s progress for all current patients in assigned unit(s).
Transfer to another unit/location and ensure smooth hand-off to receiving patient liaison in new location.
Help communicate discharge status and ensure family is prepared.
Notify patient’s shift primary nurse for family communication requests with provider.
