Job Title: LPN-LTC (Licensed Practical Nurse - Long Term Care)
Job Summary: The LPN-LTC will provide quality nursing care to residents in a long-term care facility under the guidance of registered nurses and physicians. They will assist in maintaining a safe and comfortable environment for residents, administer medications, and provide support with daily living activities.
Responsibilities and Duties:
1. Administer medication and treatments as prescribed by the physician.
2. Monitor residents' health conditions and update care plans as needed.
3. Provide assistance with activities of daily living, such as bathing, dressing, and eating.
4. Collaborate with RNs and other healthcare professionals to ensure coordinated care.
5. Respond to emergencies and provide first aid as necessary.
6. Maintain accurate and detailed medical records.
7. Educate residents and their families on healthcare needs and treatment plans.
8. Follow all state and federal regulations regarding nursing care in long-term care facilities.
Qualifications and Skills:
1. Current LPN license in the state of practice.
2. Minimum of 1 year of experience in long-term care or a related setting.
3. CPR certification.
4. Strong communication and interpersonal skills.
5. Ability to work in a fast-paced environment and prioritize tasks effectively.
6. Knowledge of infection control practices and other healthcare regulations.
7. Compassionate and patient-focused approach to care.
8. Ability to work independently and as part of a team.
