Job Title: Registered Nurse - Long Term Care (RN-LTC)
Job Summary:
The Registered Nurse in Long Term Care (RN-LTC) provides nursing care to elderly and long-term care residents in a skilled nursing facility or nursing home setting. They assess patient health needs, develop care plans, administer medications, and collaborate with other healthcare professionals to provide comprehensive care to residents.
Responsibilities and Duties:
1. Conduct comprehensive assessments of residents' health status
2. Develop individualized care plans for residents based on assessment findings
3. Administer medications and treatments as prescribed
4. Monitor residents' vital signs and condition, and report any changes to the healthcare team
5. Provide emotional support and education to residents and their families
6. Collaborate with physicians, therapists, and other healthcare professionals to coordinate care plans
7. Maintain accurate and up-to-date medical records
8. Follow facility policies and procedures to ensure resident safety and well-being
Qualifications and Skills:
1. Valid RN license in the state of practice
2. Minimum of 2 year of experience in long term care
3. Basic Life Support (BLS) certification
4. Strong clinical assessment and critical thinking skills
5. Must be living at a permanent residence at least 100 miles away from the facility.
6. Ability to work in a fast-paced environment and collaborate with a multidisciplinary team
7. Computer proficiency for maintaining electronic medical records
