Job Title: LVN/LPN - Long Term Care
Job Summary:
As an LVN/LPN in long term care, you will be responsible for providing quality nursing care to residents in a long-term care facility. You will work under the supervision of a registered nurse or physician to assess patient needs, develop care plans, administer medications, and provide emotional support to residents and their families.
Responsibilities and Duties:
1. Perform assessments of residents' health status
2. Develop and implement individualized care plans
3. Administer medications and treatments as prescribed
4. Monitor vital signs and report any changes to the healthcare team
5. Provide emotional support to residents and their families
6. Assist with activities of daily living, such as bathing, dressing, and eating
7. Collaborate with interdisciplinary team members to ensure quality care for residents
8. Maintain accurate and up-to-date documentation of resident care
Qualifications and Skills:
1. Current LVN/LPN license in the state of employment
2. Previous experience in long-term care preferred
3. Ability to work effectively in a team environment
4. Excellent communication and interpersonal skills
5. Strong attention to detail and organizational skills
6. Compassion and empathy for residents and their families
