Job Description

Job Title: LPN Wound Care

Job Summary:

The LPN Wound Care is responsible for providing specialized wound care treatment to patients under the guidance of a physician. This role involves assessing, evaluating, and treating various types of wounds to promote healing and prevent infection.

Responsibilities and Duties:

- Conduct comprehensive assessments of patients with wounds, including documenting size, depth, and appearance

- Develop and implement individualized wound care treatment plans in collaboration with the healthcare team

- Administer medications, dressings, and perform wound debridement as ordered by the physician

- Monitor and evaluate the progress of wound healing, adjusting treatment plans as necessary

- Educate patients and their families on proper wound care techniques and prevention strategies

- Maintain accurate and up-to-date patient records and documentation

- Collaborate with other healthcare professionals to ensure optimal patient care and outcomes

EXPERIENCE/EDUCATION/QUALIFICATIONS

Current unencumbered Oregon LPN Licensure required upon submission.
Healthcare Provider Level AHA BLS required.
PCH Wound Ostomy:
- Previous experience in a clinical setting and/or wound care preferred.
