Job Title: Licensed Practical Nurse (LPN) – Hospice
Job Summary:
As a Licensed Practical Nurse (LPN) working in hospice care, you will provide compassionate and holistic nursing care to patients at the end-of-life stage. You will work closely with a team of healthcare professionals to ensure the comfort and well-being of patients and their families during this difficult time.

Hospice Licensed Practical Nurses at the VNH are an integral part of the care team, where they assess a patient’s health status and complete nursing interventions in collaboration with the interdisciplinary team members and the patient. As a member of the hospice team, the Licensed Practical Nurse (under the direction of an RN Case Manager) is responsible for direct nursing care to Hospice patients and for educating patients and families in accordance with the care plan established by the interdisciplinary team.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by the hospice physician.
2. Monitor and assess patient symptoms and changes in condition.
3. Provide emotional support and guidance to patients and their families.
4. Collaborate with the hospice interdisciplinary team to develop and implement individualized care plans.
5. Educate patients and families on end-of-life care and coping strategies.
6. Assist with pain management and symptom control.
7. Help with personal care, such as bathing, dressing, and feeding.
8. Maintain accurate and up-to-date patient records.
9. Coordinate with other healthcare providers to ensure continuity of care.
Qualifications and Skills:
1. Graduation from an accredited LPN program.
2. Must be a Licensed Practical Nurse in Vermont and New Hampshire.
3. Minimum of 1 year of nursing experience, preferably in hospice or palliative care.
4. Strong communication and interpersonal skills.
5. Compassionate and empathetic demeanor.
6. Ability to work effectively as part of a team.
7. Knowledge of end-of-life care practices and principles.
8. Basic computer skills for documentation and communication purposes.
9. Current CPR certification.
