Job title: Social Worker Hospice
Job Summary:
The Social Worker Hospice is responsible for providing emotional support, counseling, and advocacy to patients and their families who are facing end-of-life care. They work closely with healthcare professionals to ensure holistic care is provided to individuals in hospice settings.
Responsibilities and Duties:
- Conduct initial assessments of patients and families to determine their psychosocial needs
- Develop individualized care plans in collaboration with healthcare team
- Provide counseling and emotional support to patients and families
- Coordinate community resources and facilitate referrals for additional support
- Advocate for patients' rights and ensure their wishes are respected
- Assist with discharge planning and coordination of hospice care services
- Document patient progress and maintain accurate records
Qualifications and Skills:
- Bachelor's or Master's degree in Social Work
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred
- Experience working in hospice or palliative care settings
- Strong communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Knowledge of community resources and support services
- Compassionate and empathetic attitude towards patients and families
