Job Title: Home Health RN
Job Summary:
The Home Health RN is responsible for providing quality and compassionate care to patients in their homes. This includes assessing patient needs, developing care plans, administering medications, and educating patients and their families on proper health management.
Responsibilities and Duties:
1. Conduct initial assessments of patients' health and develop individualized care plans
2. Administer medications and treatments as prescribed by the physician
3. Monitor patients' progress and adjust care plans as needed
4. Educate patients and their families on proper health management and self-care techniques
5. Collaborate with other healthcare professionals to ensure continuity of care
6. Document patient interactions and progress in electronic medical records
7. Participate in team meetings and communicate regularly with supervisors
8. Provide emotional support and companionship to patients in their homes
Qualifications and Skills:
1. Current RN license in the state of practice
2. Minimum of 1 year of experience in home health or a related field
3. CPR certification
4. Strong clinical assessment skills
5. Excellent communication and interpersonal skills
6. Ability to work independently and prioritize tasks
7. Proficient in using electronic medical records systems
8. Valid driver's license and reliable transportation
