Job Title: RN Case Manager
Job Summary:
We are seeking a motivated and experienced Registered Nurse (RN) to join our team as a Case Manager. The RN Case Manager will be responsible for coordinating and managing the care of patients within our healthcare facility. This position requires strong clinical skills, the ability to assess patient needs, and excellent communication and organizational skills.
Responsibilities and Duties:
- Conduct initial assessments of patients to determine their individual needs and develop personalized care plans
- Coordinate with healthcare providers, social workers, and other members of the healthcare team to ensure seamless care for patients
- Monitor and evaluate patient progress and adjust care plans as needed
- Provide education and support to patients and their families on medical conditions, treatment options, and self-care strategies
- Maintain accurate and up-to-date documentation on patient care and progress
- Collaborate with insurance providers and other agencies to coordinate services for patients
- Participate in quality improvement initiatives to ensure the highest standard of care for patients
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 2 years of clinical experience in a healthcare setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of healthcare regulations and standards
- Proficiency in electronic medical records systems
