Job Title: Licensed Practical Nurse - Long-Term Care (LPN-LTC)
Job Summary:
The Licensed Practical Nurse - Long-Term Care (LPN-LTC) is responsible for providing direct nursing care to residents in a long-term care facility. They work under the supervision of registered nurses and physicians to ensure the health and well-being of residents.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and report changes in resident's condition to healthcare team
- Assist residents with activities of daily living, such as bathing, dressing, and eating
- Document all care provided in resident's medical records
- Collaborate with interdisciplinary team to develop and implement care plans
- Provide emotional support and education to residents and their families
- Adhere to all federal, state, and facility regulations and policies
- Maintain a safe and clean environment for residents
Qualifications and Skills:
- Current LPN license in state of practice
- Previous experience in long-term care or geriatric nursing preferred
- Strong communication and interpersonal skills
- Ability to work well in a team environment
- Basic computer skills for documenting care provided
- BLS certification required
