Job Title: LPN-LTC (Licensed Practical Nurse - Long Term Care)
Job Summary:
As an LPN-LTC, you will be responsible for providing personalized nursing care to residents in a long-term care facility. Your primary role will be to assess, plan, implement, and evaluate the nursing care of residents while ensuring their safety, comfort, and well-being.
Responsibilities and Duties:
1. Conducting initial resident assessments and developing care plans.
2. Administering medications and treatments as prescribed by healthcare providers.
3. Monitoring residents' vital signs and condition, and reporting any changes to the nursing team.
4. Assisting with activities of daily living, such as bathing, dressing, and grooming.
5. Collaborating with other healthcare professionals to provide comprehensive care to residents.
6. Educating residents and their families on health management and prevention.
7. Maintaining accurate and up-to-date medical records and documentation.
8. Adhering to facility policies, procedures, and regulatory guidelines.
Qualifications and Skills:
1. Valid LPN license in the state of practice.
2. Prior experience in long-term care or geriatric nursing preferred.
3. Strong communication and interpersonal skills.
4. Ability to work effectively in a fast-paced and dynamic environment.
5. Compassion, empathy, and a patient-centered approach to care.
6. Excellent organizational and time management skills.
7. Knowledge of nursing best practices and regulations.
8. CPR certification.
