Job Description:

Responsibilities:

• Provide holistic nursing care to patients receiving hospice services, focusing on pain and symptom management, comfort, and quality of life.
• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs, collaborating with the interdisciplinary team to develop individualized care plans.
• Administer medications and treatments as prescribed, ensuring patient comfort and dignity.
• Educate patients and their families about the hospice philosophy, disease process, symptom management, and end-of-life care options.
• Offer emotional support and counseling to patients and families, fostering a compassionate and supportive environment.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, and other members of the interdisciplinary team to ensure continuity of care.
• Document patient assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards.

Requirements:

• 2 years of Hospice case management experience required
• Acute/Med Surg experience preferred
• BLS (AHA) - required

Pay Package:

• Hourly Rate: $60 per hour

Shift Timing: 8:00 AM - 5:00 PM
