Shift: Night Shift(6PM - 6AM)

Duration: 13 Weeks
Job Summary:
The LPN Post Acute LTC is responsible for providing direct patient care in a post-acute long term care setting. This includes assisting patients with activities of daily living, administering medications, and collaborating with the healthcare team to ensure the best possible outcomes for patients. The LPN Post Acute LTC will also be responsible for documenting patient progress and communicating with patients and their families.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Assist patients with personal care tasks, such as bathing and dressing
- Monitor patient vital signs and report any changes to the healthcare team
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans
- Educate patients and their families on care plans and health maintenance
- Document patient assessments, interventions, and outcomes accurately and timely
- Adhere to all healthcare policies and procedures to ensure patient safety and well-being
Qualifications and Skills:
- Must be a licensed practical nurse in the state of [State]
- Previous experience in a long term care or post-acute care setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail and documentation skills
- BLS License Required
- Basic computer skills for electronic charting
