**Job Title:** Case Manager
Shift: 8hrs
Location: Sitka AK
Duration: 13 weeks
**Job Summary:**
A case manager is responsible for overseeing and coordinating the care of clients who require social services. They work closely with clients, healthcare professionals, and social service providers to ensure that clients receive the appropriate support and resources they need to improve their quality of life.
**Responsibilities and Duties:**
- Conduct assessments to determine the needs of clients
- Develop individualized care plans for clients
- Coordinate services such as medical care, housing, and transportation for clients
- Monitor clients' progress and adjust care plans as needed
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date client records
- Collaborate with other healthcare professionals and social service providers
- Provide crisis intervention and support to clients in need
**Qualifications and Skills:**
- BLS and CCMA required
- Bachelor's degree in social work, psychology, or related field
- Previous experience in case management or social services
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Knowledge of community resources and social service programs
- Excellent organizational and time management skills
- Ability to work independently and make sound decisions
