Job Summary:
Responsible for planning, implementing, and evaluating physical therapy services for patients in concert with the total care plan. Provide quality care to attain patient’s optimal functional level.
Job Duties and Responsibilities
1. Provides comprehensive Physical Therapy care in both outpatient and inpatient settings. Performs intakes and evaluations, discusses goals with patients, implements care plans, prescribes medications, educates patients about risks and benefits, and monitors progress. Works closely with other members of patient care team in the formulation of the total care plan for patients.
2. Utilizes assessment data to develop a care plan based on the problem-oriented record format: develop a written treatment plan, including objectives and methods of teaching and adjust information to patient’s current level of understanding: prepare and instruct PT approaches; adjust patient care plan as appropriate.
3. Assure implementation and compliance of the therapy plan. Modify treatment plan as indicated by patient condition progress. Instruct caregivers and family for maximal function and safety in the home environment, including proper use of equipment, body mechanics and guarding techniques. Instruct, supervise and counsel caregivers and family members in continuity of care management. Make referrals to other disciplines as appropriate.
4. Evaluate the therapy plan. Review the written plan and its effect on patient care and make appropriate modifications at least every 30 days. Obtain feedback from the other team members regarding the plan’s effectiveness. Explore with the staff, patient and /or family, progress in care. Consult with the attending physician concerning alteration of the therapy plan. Review patient’s record in appropriate utilization review setting. Meet as needed with the Rehabilitation Manager regarding patient care problems and how these may be resolved.
5. Meets or exceeds quality care standards, achieves clinical excellence, contributes to quality improvement, and strives for high patient satisfaction. Independently plans work and develops procedures to accomplish goals. Participates actively in the quality improvement system, complies with all appropriate Washington State Department of Health standards, National Patient Safety goals, and other quality improvement projects.
6. Maintains professional skills through research and training/continuing education opportunities. Participates in interdisciplinary meetings; stays abreast of trends and best practices; develops and implements new and revised methods, practices, policies, techniques and procedures. Ensures compliance with all regulatory requirements and federal, state and local laws.
7. Special projects and other related duties as assigned
Required Education and Experience
· Clinical doctorate, Master’s or Bachelor’s degree in Physical Therapy
· One year of physical therapy practice, preferred
Required Licenses, Certifications and/or Registrations
1. Current and unrestricted license to practice Physical Therapy in the state of Washington
2. Current BLS certification

1. Comprehensive knowledge and understanding of therapy theories and concepts.
2. Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience.
3. Ability to cultivate effective partnerships and work in collaboration with providers, other health care professionals, managers, and patients.
4. Ability to maintain accurate and timely documentation and records.
5. Ability to work independently performing a wide variety of duties that require the use of independent judgment, advanced problem-solving, critical thinking, organization, and prioritization skills.
