Job Title: LPN - Long Term Care (LTC)
Job Summary:
As an LPN working in Long Term Care (LTC) facilities, you will be responsible for providing direct patient care to residents, collaborating with healthcare providers, and ensuring the overall well-being of the elderly population in a long-term care setting.
Responsibilities and Duties:
- Administer medications to residents as prescribed by physicians
- Monitor residents' vital signs and report any changes to the nursing team
- Assist residents with activities of daily living, such as bathing, dressing, and grooming
- Collaborate with other healthcare providers to create individualized care plans for residents
- Educate residents and their families on health-related topics and self-care techniques
- Maintain accurate and up-to-date patient records and documentation
- Adhere to all regulations and guidelines set forth by the LTC facility and regulatory agencies
Qualifications and Skills:
- Valid LPN license in the state of practice
- Minimum of 1 year of experience working in a long-term care setting
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Strong attention to detail and organizational skills
- Compassionate and empathetic attitude towards elderly patients
- Knowledge of Medicare and Medicaid regulations
