Job Title: Case Manager
Shift: 8:00 AM - 4:30 PM
Location: New London, NH
Duration: 13 Weeks
Job Summary:
The Case Manager is responsible for providing holistic, client-centered support to individuals in need by assessing their needs, developing care plans, coordinating services, and advocating on their behalf. They work closely with clients, families, and service providers to ensure that the client's needs are being met and that they are receiving the necessary resources and support.
Responsibilities and Duties:
1. Conduct comprehensive assessments of clients' needs and develop individualized care plans.
2. Coordinate and monitor services and resources for clients, ensuring that they receive appropriate care and support.
3. Advocate for clients and ensure that their rights and preferences are respected.
4. Collaborate with other professionals, such as social workers, healthcare providers, and community organizations, to provide comprehensive care to clients.
5. Monitor and evaluate clients' progress, adjust care plans as needed, and provide ongoing support.
6. Maintain accurate and up-to-date records of clients' information and services provided.
7. Participate in case conferences and team meetings to discuss client progress and develop strategies for ongoing support.
Qualifications and Skills:
1. Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).
2. Previous experience working as a case manager or in a related role.
3. Strong communication and interpersonal skills.
4. Ability to work effectively with diverse populations.
5. Knowledge of community resources and services.
6. Ability to work independently and as part of a team.
7. Excellent organizational and time management skills.
