Job Title: RN Clinical Care Coordinator
Shift: 7:00 AM – 3:00 PM
Location: Albuquerque, NM
Duration: 13 Weeks
Job Summary:
The RN Clinical Care Coordinator is responsible for overseeing and coordinating patient care within a healthcare facility. This position involves working closely with patients, families, healthcare providers, and other team members to ensure the highest level of care and support is provided throughout the patient's treatment journey.
Responsibilities and Duties:
1. Develop and implement individualized care plans for patients
2. Coordinate and facilitate communication between healthcare providers and patients
3. Monitor and evaluate patient progress and make necessary adjustments to care plans
4. Provide education and support to patients and their families
5. Collaborate with multidisciplinary teams to ensure seamless care coordination
6. Assist in discharge planning and transitioning patients to post-acute care settings
7. Advocate for patient needs and ensure quality of care is maintained
8. Maintain accurate and up-to-date patient records and documentation
9. Participate in quality improvement initiatives and performance improvement projects
10. Stay current with best practices and evidence-based guidelines in patient care delivery
Qualifications and Skills:
1. Valid RN license in the state of practice
2. Bachelor's degree in Nursing preferred
3. Minimum of 2 years of clinical experience in a healthcare setting
4. Strong communication and interpersonal skills
5. Ability to multitask and prioritize effectively
6. Knowledge of healthcare regulations and compliance standards
7. Experience working with electronic health record systems
8. Critical thinking and problem-solving skills
9. Ability to work independently and as part of a team
10. BLS certification required
