Job Title: Case Manager
Location: Mt Vernon, WA
Shift: 8-12 hrs
Duration: 13 Weeks
Job Summary:
As a Case Manager, you will be responsible for coordinating and facilitating services for individuals in need of support, guidance, and resources. You will work closely with clients, families, and other support systems to develop comprehensive care plans and ensure that all needs are met.
Responsibilities and Duties:
- Conduct initial assessments to determine client needs and develop individualized care plans
- Coordinate services and resources to support clients in achieving their goals
- Advocate for clients and provide support in navigating healthcare, social services, and other systems
- Monitor progress and make adjustments to care plans as needed
- Collaborate with other professionals, such as social workers, therapists, and healthcare providers, to ensure comprehensive care
- Maintain accurate and up-to-date client records and documentation
- Provide crisis intervention and support in emergency situations
- Participate in case conferences and team meetings to discuss client progress and make recommendations for ongoing support
Qualifications and Skills:
- Bachelor's degree in social work, psychology, counseling, or related field
- Previous experience working in a case management role or with vulnerable populations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and social services
- Compassion and empathy for clients facing challenges
- Strong organizational and time management skills
