Job Title: LPN / T-LPN - DE

Job Summary:

We are seeking a talented and compassionate LPN / T-LPN to join our healthcare team in Delaware. The ideal candidate will be responsible for providing patient care under the direction of a registered nurse or physician. This role requires excellent communication skills, attention to detail, and a genuine desire to help others.

Responsibilities and Duties:

- Administering medications and treatments as prescribed by healthcare providers

- Monitoring patient vital signs and reporting any abnormalities to the healthcare team

- Assisting with patient assessments and developing care plans

- Providing patient education on treatment plans and self-care techniques

- Collaborating with other healthcare professionals to ensure high-quality patient care

- Maintaining accurate and up-to-date patient records

- Following all safety protocols and infection control practices

Qualifications and Skills:

- Current LPN license in the state of Delaware

- Previous experience in a healthcare setting preferred

- Excellent communication and interpersonal skills

- Strong attention to detail and ability to multitask

- Ability to work effectively in a fast-paced environment

- Basic computer skills for electronic health record documentation
