Job Title: RN LTC-SS-HI
Job Summary:
As a Registered Nurse (RN) in a Long-Term Care (LTC) facility with a Special Skills (SS) focus on Hospice and Palliative Care (HI), you will be responsible for providing comprehensive nursing care to residents with chronic illnesses and end-of-life needs. You will work closely with interdisciplinary teams to develop and implement individualized care plans, manage symptoms, and provide emotional support to residents and their families.
Responsibilities and Duties:
1. Assess resident health needs and create care plans in collaboration with the healthcare team.
2. Administer medications, treatments, and procedures as prescribed by physicians.
3. Monitor resident vital signs and symptoms, and report any changes to the healthcare team.
4. Provide emotional support and comfort to residents and their families during difficult times.
5. Educate residents and families on disease management, treatment options, and end-of-life care.
6. Collaborate with social workers, therapists, and other healthcare professionals to ensure holistic care for residents.
7. Maintain accurate and timely documentation of all care provided.
Qualifications and Skills:
1. Licensed as a Registered Nurse (RN) in the state of practice.
2. Minimum of 2 years of experience in long-term care or hospice settings.
3. Knowledge of palliative and end-of-life care principles and practices.
4. Strong communication and interpersonal skills.
5. Ability to work effectively in a team-oriented environment.
6. Compassionate and patient-centered care approach.
7. BLS certification required, ACLS certification preferred.
