Job Title: RN LTC-WI-SS (Long-Term Care)
Job Summary: As an RN in a long-term care facility in Wisconsin, you will be responsible for providing comprehensive nursing care to residents in a skilled nursing setting. Your duties will include assessing patient needs, developing care plans, administering medications, and collaborating with other healthcare professionals to ensure the highest quality of care for residents.
Responsibilities and Duties:
1. Conduct nursing assessments and develop individualized care plans for residents
2. Administer medications and treatments according to physician orders
3. Monitor and evaluate patient responses to treatments and interventions
4. Coordinate care with interdisciplinary team members, including physicians, therapists, and social workers
5. Provide education and support to residents and their families
6. Assist with admissions, discharges, and transfers of residents
7. Maintain accurate and up-to-date medical records
8. Adhere to all state and federal regulations regarding long-term care facilities
Qualifications and Skills:
1. Valid RN license in the state of Wisconsin
2. Minimum of 2 years of experience in long-term care or a similar setting
3. BLS certification
4. Strong clinical assessment and critical thinking skills
5. Excellent communication and interpersonal abilities
6. Ability to work independently and as part of a team
7. Knowledge of Medicare and Medicaid regulations
8. Proficiency in electronic medical records systems
