Job Title: RN Case Manager
Job Summary:
The RN Case Manager is responsible for coordinating and providing patient-centered care, ensuring that each patient receives the appropriate level of care and support. They work closely with physicians, healthcare providers, and other members of the healthcare team to develop and implement individualized care plans that meet the needs of each patient.
Responsibilities and Duties:
1. Assessing patients' medical needs and developing care plans in collaboration with other healthcare professionals.
2. Coordinating patient care and services, including arranging for medical procedures, prescriptions, and equipment.
3. Monitoring patients' progress and adjusting care plans as needed.
4. Providing education and support to patients and their families regarding treatment options, medications, and health management.
5. Collaborating with insurance companies and healthcare facilities to facilitate smooth transitions of care.
6. Advocating for patients' rights and ensuring they receive quality healthcare services.
7. Documenting patient care activities and maintaining accurate medical records.
Qualifications and Skills:
1. Current RN license in the state of practice.
2. Minimum of 2 years of clinical nursing experience, preferably in a case management setting.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a multidisciplinary team.
5. Knowledge of healthcare regulations and insurance policies.
6. Excellent organizational and time management skills.
7. Proficiency in electronic medical records (EMR) systems.
