Position: Case Manager/Discharge Planner/Utilization Review Location: Meeker, CO 81641 Shift: Monday to Friday, 8:00 AM to 5:00 PM Salary: $58-$68 per hour

Responsibilities:

• Collaborate with healthcare providers to assess, plan, implement, coordinate, monitor, and evaluate options and services to meet the patient's health care needs.
• Conduct utilization reviews to ensure appropriate allocation of resources and services.
• Coordinate discharge planning processes to facilitate smooth transitions for patients from hospital to home or other care settings.
• Advocate for patients and their families to ensure access to necessary resources and support services.
• Maintain accurate and up-to-date documentation of patient assessments, care plans, and interactions.

Requirements:

• Bachelor's degree in Nursing or related field required; Master's degree preferred.
• Current licensure as a Registered Nurse (RN) in the state of Colorado.
• Previous experience in case management, discharge planning, or utilization review preferred.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a multidisciplinary team environment.
